What Are the Correct Levels of Staffing For Senior Living Communities?

doug Fullaway Unemployment seems to be stuck around 10% and foreclosures are reaching record levels, casting a dark cloud over the business of senior living. Prospective new residents are more uncertain than ever about moving. Centers work hard just to hold census levels. And while there are signs things are improving, it is likely to be several years before there is a significant improvement. Every operator is driven to cut costs and with labor making up 60% or more of total operating expenses, it is the one thing that cannot be ignored. So chances are you have cut or will cut your labor costs. But how much can you cut and when have you cut enough?

The first step for most communities is to reduce the staff hours to some level or ratio that has worked in the past. It’s a logical place to start. This approach assumes that the level or ratio is the right level. But where did this ratio come from? In most cases, it comes from experience. And maybe that’s OK. But a closer look reveals some possible problems and opportunities. In a 2009 survey[1] we conducted of more than 90 nurses in assisted living, we found that the most common method used to arrive at staffing was a fixed ratio of number of staff per number of residents. But only 25% of those nurses felt this was the best method for staffing. The vast majority of the nurses felt that “acuity based staffing” was the best method. Why the discrepancy? The nurses told us that they just don’t have the tools to staff based on acuity and the ratio was a reasonable approximation. We decided to take a closer look to understand the consequences of staffing based on ratio instead of acuity.

A fixed ratio of caregivers to residents assumes that the services being delivered are the same over time. So we looked at the data for one community over a year, based on the acuity. This analysis could be done because this community had a standard time for every service task. For example, a task such as partial bathing assistance was given a time of 22 minutes. And each resident had an assessment that had only the service tasks that were needed by that resident. Assessments were done every 90 days and when conditions changed. We could see the acuity by resident and for the community for the entire year. When we took the actual acuity-based staffing hours and computed the actual ratio needed to provide exactly the hours needed, we arrived at a ratio of 3.25 residents/caregiver (or 0.308 if you prefer the more traditional caregivers/resident ratio). As a comparison, The State of Seniors Housing 2009[2] shows a ratio of 0.39 for comparable departments. When we applied the 3.25 ratio to the actual acuity here is what we found:

The number of hours based on ratios does rise and fall with the changes in census, but it is always higher and sometimes by quite a bit. This happens because the 3.25 residents per caregiver was selected as it matched the acuity and census average for the entire year. And when the ratio is applied to the actual census it is rounded up using the rule that if calculation showed point 5 (0.5) or more than another person was put on the shift.

If the ratio was reduced to 3.15 residents/caregiver then some of the “error” can be reduced, but the problem of overstaffing still exists because the ratio causes changes in staffing with changes in census but it ignores changes in actual acuity of the residents. Here is an example of how the acuity changed for one of the residents over time:

A fixed ratio per resident for staffing would simply ignore the changes that this resident experienced. Many communities that are using staffing ratios are most likely paying more for labor than they would if the measured the actual acuity and changed the staffing weekly.

Microsoft Virtual Senior Center Project Connects Homebound Seniors In New York City

Microsoft Corp., the city of New York and Selfhelp Community Services Inc. recently unveiled their Virtual Senior Center, a public-private partnership and demonstration project that is showing how cities can use technology to revitalize senior centers and enhance the lives of homebound seniors. The Virtual Senior Center uses computer, video and Internet technology to create an interactive experience for homebound seniors that reduces social isolation and gives them better access to community services.  The project links six homebound seniors (ranging in age from 67 to 103) to Selfhelp’s Benjamin Rosenthal Senior Center in Flushing, Queens and each home is equipped with a desktop computer as well as a touch-screen monitor, a small video camera, a microphone and broadband Internet service.

Video cameras and monitors have been strategically placed around the senior center to enable the homebound seniors to interact with classmates and instructors at the center, and to take part in activities such as armchair yoga, painting classes, current events discussions and tai chi. Using the technology, seniors at home can see and hear the other people in the class and actively participate in two-way discussions and activities. Since beginning the project, some have even made new friends. It’s Never 2 Late, a Colorado company that creates specialty technology packages for seniors, provides the custom interface.  The Virtual Senior Center project included a psycho-social assessment that measured a number of attitudes, health attributes and emotional factors for the six participating seniors at the start of the program and at various stages. As a group, the seniors showed marked improvement throughout the course of the project.

"The New York City Department for the Aging is deeply committed to improving the quality of life for older New Yorkers, and this partnership with Microsoft and Selfhelp Community Services in creating the Virtual Senior Center is one more step toward making New York City the most age-friendly city in the nation," said Lilliam Barrios-Paoli, commissioner of the New York City Department for the Aging (DFTA). "Senior centers are the social hub for many older New Yorkers, and this new model — the Virtual Senior Center — has shown us that technology will help seniors age in place and remain integrated into the community by bringing that same senior center experience into the home."

"The opportunity for homebound seniors to interact virtually with caregivers, providers, peers, family members and friends has made a significant and measurable difference in the quality of their lives," said Becky Bigio, director, Selfhelp Senior Source Geriatric Care Management Program. "Nearly all the candidates have described feeling more connected to others and show an increasing awareness and appreciation of those connections."

2010 Retirement Confidence Survey Shows Bottoming In Confidence Levels

The Employee Benefit Research Institute recently released its annual Retirement Confidence Survey for 2010 that that the record-low confidence levels measured during the past two years of economic decline appear to have bottomed out. The percentage of workers very confident about having enough money for a comfortable retirement has stabilized at 16 percent, which is statistically equivalent to the 20-year low of 13 percent measured in 2009.  One of the more troubling results of the survey shows that An increased percentage of workers report they have virtually no savings and investments. Among RCS workers providing this type of information, 27 percent say they have less than $1,000 in savings (up from 20 percent in 2009). In total, more than half of workers (54 percent) report that the total value of their household’s savings and investments, excluding the value of their primary home and any defined benefit plans, is less than $25,000.

Sentiment about having a financially secure retirement has also stabilized according to the survey.  Some of the other notable statistics include:

  • 29 percent now saying they are very confident about having enough money to pay for basic expenses during retirement (up from 25 percent in 2009, but still down from 34 percent in 2008)
  • Just 19 percent of workers and 22 percent of retirees report they are very confident about banks, while 12 percent of workers and 13 per-cent of retirees say they are very confident about insurance companies (Fig. 19, pg. 19). They are most likely to express confidence in private employers (23 percent of workers and 27 percent of retirees very confident) and least likely to feel confidence in the federal government (11 percent of workers and 8 percent of retirees).

Provident Crossing Celebrates Ground Breaking of $22.5 Million Project in Round Rock, Texas

Provident_Crossings_rendering Resort Lifestyle Communities celebrated the ground breaking of Provident Crossings adult living community in Round Rock, Texas last week. Provident Crossings is a $22.5 million, 126-apartment adult living community for people 55 and and will offer a selection of studio, 1, 2, and 3 bedroom apartments all with full kitchens, washer/dryer hookups, spacious living areas, and storage. The project was designed and will be built by Cameron General Contractors and will be managed by Resort Lifestyle Communities, both from Lincoln, Nebraska.

North Carolina CCRC Announces Large Solar Thermal Project

Friends Homes Inc. (Friends), a continuing care retirement community (CCRC) operator in North Carolina, recently announced a partnership with FLS Energy for installation of 208 solar thermal collector panels in its two continuing care retirement communities, Friends Homes at Guilford and Friends Homes West.  As one of the largest ever installed solar thermal systems in a retirement community in the United States, the project will create 10,000 gallons of solar heated water each day for nine of Friends Homes’ buildings and facilities.   The solar thermal facilities will be roof mounted to the selected facilities requiring about 12,480 sq. ft. of roof space and will reduce carbon emissions by almost 150 tons per year and will save approximately $30,000.00 per year in fossil fuel cost. Construction is slated to begin in April with completion projected for September.  FLS Energy is financing the solar thermal project at Friends Homes through its Solar Energy Purchase Agreement.

The solar project, which will be metered, comes with a “performance guarantee” from FLS Energy of the amount of solar energy that will be generated each year. Duke Energy is purchasing renewable energy credits from this solar energy system to meet North Carolina’s renewable energy mandates. FLS Energy will also be responsible for all maintenance of the system. Completion of the solar hot water project in September will establish Friends Homes as one of the most sustainable continuing care retirement communities in the United States.

Wilson Sheldon, CEO, of Friends Homes Inc., said, “It is our belief that we must reduce the consumption of fossil fuels. Installing these solar thermal units in our retirement communities means taking a major step in our commitment to utilizing renewable energy. Not only do we want to be good stewards of the environment, but also, we are looking for ways to contain costs so that our communities remain affordable.”

“This project will establish Friends Home as one of the strongest leaders in North Carolina for developing clean renewable energy for their facilities,” said FLS Energy’s Finance Director, Brownie Newman. “Friends Homes paid nothing upfront and will begin seeing the savings immediately.”

Healthcare REIT Gets Pricing and Triple B Rating For $300 Million Offering

Health Care REIT, Inc. (NYSE:HCN) recently announced that it has priced an offering of $300 million of 6.125% senior unsecured notes due April 15, 2020. The notes were priced at 99.297% of their face amount to yield 6.22% and the offering is expected to close on April 7, 2010.  Fitch Ratings has assigned a ‘BBB’ rating to the $300 million 6.125% senior unsecured notes and the Rating Outlook is Stable.  The company intends to use the net proceeds from this offering for general corporate purposes, including investing in health care and senior housing properties and repaying borrowings under its unsecured line of credit and other outstanding indebtedness.
